Understand the issue
A useful care plan identifies who can reach your parents, who knows their wishes and what happens when the usual contact is unavailable. Decisions should respect their consent and independence.
Prepare your next steps
Agree what support they want
Discuss visits, meals, transport, finances and contact frequency with your parents rather than making decisions for them.
Name a local contact and backup
Confirm who can visit, who has agreed access to the home and how they will be reached.
Prepare an emergency information sheet
Keep the treating doctor, preferred hospital, medication list supplied by the clinician, allergies and insurance contact in secure family storage.
Set a simple check-in routine
Agree what a missed call means and when the local contact should check in.
Review the plan after a change
Revisit it after a hospital stay, a fall, a bereavement or a change in the local support person.
